Program Purpose
This Diabetes Prevention Program is designed to help individuals take action if they are at risk for type 2 diabetes. The program is science-based and focuses on prevention through lifestyle change rather than medication alone.
Understanding Type 2 Diabetes
Type 2 diabetes is the most common form of diabetes. Much of the food we eat is converted into glucose for energy. Insulin, produced by the pancreas, helps move glucose into the body’s cells. When the body does not use insulin effectively, glucose builds in the bloodstream.
Type 2 diabetes is a serious condition and can lead to complications such as heart attack, stroke, blindness, kidney failure, or loss of toes, feet, or legs.
Risk Assessment:
A risk test is available to help individuals better understand their likelihood of developing type 2 diabetes. The assessment also provides access to five simple steps that can help support diabetes prevention.
Prediabetes and Risk Factors
Many people are at risk for prediabetes and type 2 diabetes without realizing it. Risk increases if you:
- Are 45 years of age or older
- Have a family history of type 2 diabetes
- Are physically active fewer than three times per week
- Had gestational diabetes or delivered a baby weighing more than nine pounds
